WP 1 - Structuring data collection and aggregation of transdisciplinary modulators of disease trajectories
One of the most important shortcomings in multimorbid chronic diseases management and research is the lack of integration of social, societal and behavioral factors into the prediction of patient’s treatment responses and outcomes.
To overcome this, we will aim at aggregating information on environmental exposures and factors contributing to sedentary behavior, poor nutrition and access to care. This requires breaking “silo culture” to combine data, identify most relevant information and stimulate and systematize common prospective data collections among partners.


  • Share existing datasets of MCD trajectories modulators to feed in a unique processing framework
  • New campaigns of data collections involving transdisciplinary partners
  • Make available new markers of diseases trajectories
  • Overlap mapping of transdisciplinary modulators to better characterize disease trajectories


  • Identify and combine existing structured datasets
  • Harvest new data sources for describing MCD trajectories
  • Identify and test evidence scenarios recapitulating MCD trajectories
  • Organize and support transdisciplinary campaigns of data collection
